Information we collect on this website
- Information you send us. If you use the call-back form, we receive what you enter: your name and phone number, and, if you choose to give them, your city, the type of insurance you have, and a few words about what you need. Please keep health details to a minimum in the form; we will talk by phone.
- Technical information. Like most websites, our hosting provider may record basic technical data such as your IP address, browser type, pages viewed, and the date and time of your visit, in server logs used for security and troubleshooting.
- Fonts. This site loads fonts from Google Fonts. Your browser requests those files from Google, which may record your IP address. See our Cookie Policy.
- Analytics and advertising cookies, only if you accept them. With your consent, this site uses Google Analytics to understand how visitors find and use the site, and Google Ads measurement to see whether our ads lead to calls or call-back requests. These tools set cookies and send your IP address, device details, and the pages you view to Google. We never send your name, your phone number, or anything you type into the form. You can decline when you first visit and change your mind at any time with the "Your privacy choices" link at the bottom of every page.
We do not sell personal information. If you accept advertising cookies, Google may use them to show our ads to you on other websites. California law calls this "sharing." You can opt out at any time with the "Your privacy choices" link at the bottom of every page. We also treat the Global Privacy Control signal and the "Do Not Track" setting from your browser as an opt-out: when either is on, no analytics or advertising cookies are set, even if you accepted earlier.
How we use information
- To call you back when you ask us to, and to text you only if you checked the box on the form allowing texts. Reply STOP to any text to end them.
- To keep the website secure and working.
- To meet legal obligations.
How we share information
Call-back requests go straight to our own Google Workspace account, which is covered by a HIPAA business associate agreement with Google. No other company handles the form. We share information with service providers only as needed to operate this website, and with authorities when the law requires it. We do not give your name or phone number to advertisers. If you accept advertising cookies, Google receives the browsing information described in our Cookie Policy, which California law calls "sharing"; you can opt out at any time.
Your choices
Cookies for analytics and advertising are off until you accept them. To change your choice, use the "Your privacy choices" link at the bottom of any page. You can also opt out of Google Analytics with Google's browser add-on and manage ad personalization in Google's My Ad Center.
You can call us instead of using any form. You can ask us to delete a call-back request by emailing info@cchomehealthinc.com. If you live in California, you have the right to know what personal information we collect about you, to ask us to delete or correct it, to opt out of the sharing of your information for advertising, and not to be treated differently for using these rights. To use any of them, email info@cchomehealthinc.com or call (818) 579-4012, and we will verify your request and respond within the time the law allows. You may also use an authorized agent.
How long we keep information
We keep a call-back request only as long as we need it to reach you and to record what your care requires; the request itself is then deleted. Our hosting provider keeps server logs for a short period for security. If you accept analytics cookies, Google keeps that data under the shortest retention period Google offers, and we can delete it on request.
How we protect information
Every page and the call-back form use an encrypted connection. Only staff who need to reach you can read call-back requests, and our email runs on a business service with security controls. No website can promise perfect security, which is one reason we ask you not to send health details through the site.
Children
This website is not directed to children under 13, and we do not knowingly collect their information.
Changes
We may update this policy. The effective date at the top will change when we do.
